Danay Gonzalez

Danay Soler Gonzalez was born and raised in Havana, Cuba and came to US when she was 22.

She completed undergrad school  in Florida Career College, Miami FL
She then relocated to Chicago and  earned a Bachelor of Science in Nursing at Chamberlain University, and also completed a Master of Science in Nursing  Family Nurse Practitioner.
The is an ANCC Board Certified Family Nurse Practitioner and has 10+ years of healthcare experience since 2011.
Her RN experience includes Chronic Acute Unit Department, Telemetry/Stroke/Medical Surgical Floor, Home care, ED.
She started working as APRN in Chicago back on 6/2021 as a transition of care NP. One of the things she loved about this job was that she was in charge of identifying and tracking root cause analyses for patients with a 30-day readmission to the hospital in order to address programmatic and system-wide improvements.
Post discharge  follow up was focus on medication reconciliation and adherence, management of patient’s quality of life and functionality, management of both acute and chronic disease states, identification and rectifying gaps in care, assessment, and support of patient’s ability to perform self-cares, coordination of post-discharge appointments and services (durable medical equipment, home health), and coordination of care across the care continuum.
She relocated to Jacksonville from Chicago on 5/2022 and started working with Dr. Castello (Cardiology NP) on 6/2022. Cardiology has always been my passion.
She loves spending time with family and loves to travel.
