Blue Cross Blue Shield Members: Rate our Doctors

We are proud that many of our doctors have earned the Blue Distinction Total Care honor from Blue Cross Blue Shield. Blue Distinction Total Care is a national program that recognizes doctors who take the time to foster strong relationships with their patients. The program honors physicians who keep prevention, personalized care and patient care needs at the forefront of their practice.

1. Visit the Blue Cross Blue Shield website
2. Log in by clicking the green button in the right-hand corner
3. Click on “Tools”
4. Click on “Find a Doctor or Hospital”
5. Search for your physician by last name and select your physician at the location of your appointment
6. Click on “Rate This Doctor”
7. You will then be directed to a third party website. Click “Yes, take me to the site” to be redirected.
8. You will be prompted to enter the three letter code of your member number which can be found on the front of your insurance card.
9. Give your First Coast Cardiovascular Institute & Jacksonville Clinic doctor a 5 star rating!
